You must have a version of Heretic II greater than 1.0 to use custom skins in a single-player game.
Unzip these skins into your /Heretic II/base/ players/male/ directory. For deathmatch or coop games, these skins can be selected from the Multiplayer - Player Config menus in the game.
In a single-player game, open the console and type:
allowillegalskins 1
skin [skinname]
where [skinname] is the name of the skin you want.
